It was set up in such a way that it was basically spoiled before they got to the end, so I figured it out but it wasn't done fair and square.

the hospital is what the car crashed into and the scene is the hospital and the surgeon is the dad BUT! yeah he was told to perform the surgery on his son and it gave him a heart attack cause he didn't realize his son was in the car somehow
