Just on a purely surface level the biggest reach of its aesthetics probably came from Kylie in 2004 given Madonna reached more for Victorian garb but I have no idea what she's even getting at here. I don't even think she does either.

What I got from that is that Miley is saying there's more to being a showgirl than what people see which is just the flashy outfits. She's probably applying it to herself as a popstar where she's suggesting that people may see popstars as vapid but there's a lot more to them than just catchy songs or looking beautiful like how Miley is genuinely a talented performer.
