I see filler which is giving him that apple-cheek effect. Either way, the man looks better than nearly any 42 year old out there.

Clearly some filler but it's honestly not bad, imho. Maybe it needs to settle.

 

It's much easier to tell on guys. I've seen worse, starting with Brad Pitt. This isn't remotely Zac Efron territory.

 

For 42 he looks fine, idk. White men age faster in the face too. People here have crazy high standards. Imho.

He is naturally good looking but I think he needs darker hair on top and darker facial hair. Really completes his face. He is very sexy at his peak so this is no shade to his natural looks. 
 

This blonde hair, Botox’d, light facial hair looks unnatural on him.  I also think he was cast way too old to play a realistic Ken. The doll is clearly modeled after a man in his 20s.

nnn, I was expecting a Zac Efron sized mess but he doesn't look all that peculiar. Still a strange choice for a 40+ year old white man in Hollywood to get botox. They are almost never hurting for roles and mass marketing no matter how old they look. Ryan isn't even that old and I don't think he looked bad before.
