I've done multiple prostate exams in my training. Do not douche before. Ideally be "empty" but 100% of doctors expect there to be some faecal material on their glove at the end of the exam. It's literally one of the steps of the exam to assess what is left on the glove, be that faeces, pus, blood or whatever else. If a patient were to clean "completely" it could lead to the practitioner missing a crucial finding.
