Sigh. As predicted.

Hopefully booster available within a year, if we're lucky.

Same :rip: I didn't follow through on the second dose because cases were going down

Oh God I hope this **** calms. The amount of racism and homophobia this will bring. Don't get me started on the actual harm of virus.

I was horrified by this in 2022 now it is all coming back to real.

Anyone in a country fortunate enough to have Jynneos in stock should get BOTH doses NOW even with less effectiveness against the new clade because stockpiles are piss-poor even in first-world countries as well as turn-around time for booster stockpiles.

Even if you're not MSM find out a way to get them. It's hard but possible in some cases in the states for example.

On the topic of Clade-specific testing, I don't know how many people are aware of this interesting quirk

Clade Ib actually at first proved quite difficult to specifically identify using the most widely used Clade-specific lab test (which is able to tell Clade IIb apart from Clade Ia for example). It works by looking for one very specific gene in the genetic code of the virus which codes differently depending on the Clade. This is the kind of lab test most often used to screen for potential Clade I samples that might pop up outside of DRC.

In a kind of crazy cosmic coincidence, one of Clade Ib's mutations deleted the specific gene that the Clade-specific mpox test is designed to look for— meaning that the test isn't able to "see" Clade Ib and fails to properly identify it as being mpox.

Because this problem was not initially recognized, that test alone has been the only test performed in samples from many places around the world that were sent in to undergo Clade-specific testing— so it's theoretically possible that a sample containing Clade Ib virus has already been collected and tested somewhere in the world but returned a false negative and therefore evaded detection.

It took some time for scientists to figure this out— it caused a lot of confusion when it was first discovered. For a time, even the U.S. CDC was having difficulty devising a protocol for properly identifying it and they essentially settled on a process of elimination method that requires you to test each sample twice— the first test tells you if it's an orthopox virus or not (but not which Clade it belongs to) and then the second test is supposed to determine which Clade of mpox it belongs (but relies on the gene that Clade Ib is missing, so it can only identify Clade IIb or Clade Ia mpox samples correctly).

If the first test comes back positive and the second test comes back negative, you assume the second test is a false negative— basically the failure of the Clade-specific test allows you to deduce that your sample might be Clade Ib. Then you send it in to a more advanced lab to be fully sequenced for confirmation.

Mpox emergency response constrained by scarce financing | The Straits Times

Congo alone has said it requires 3.5 million vaccine doses, and its public health minister, Mr Roger Kamba, has put the cost at hundreds of millions of dollars. The price of the shots has been estimated at US$100 a dose, unaffordable for many countries. In a briefing on Aug 15, he urged the international community to provide assistance.

The challenge in securing money to fight the mpox outbreak is playing out against a backdrop of very limited funding for a range of deadly diseases, said Mr Peter Sands, executive director of the Global Fund to Fight Aids, Tuberculosis and Malaria.

There are "massive gaps in financing diseases that are already killing a lot more people", he noted. While more funds might be needed, he cautioned against forgetting the wider context of a shortage of funding for diseases like malaria in Congo.

In the case of mpox, one of the quickest ways to get financing where it needs to go is to repurpose funding from mechanisms that already exist, such as work on disease prevention, said Mr Sands, who was previously chief executive officer of Standard Chartered.

In Congo, large gaps remain in knowledge about the epidemiology of the disease. The declaration of a global health emergency better focuses attention on understanding the infection and how it is spreading in the country. "We don't really have a grip on it in terms of the epidemiological dynamics and this will focus attention on that quite effectively," he added.

Pakistan admits it cannot find mpox patient as tighter screening to be introduced at airports (yahoo.com)

Pakistan's health department confirmed that one case of mpox was detected in the country's northern Khyber Pakhtunkhwa province, withdrawing a previous statement that three cases of the viral infection were detected in the region this week on arrival from the UAE.

The health ministry said it had yet to confirm if the patient is infected with the new variant known as clade 1b, with the sequencing of a sample from the Khyber Pakhtunkhwa patient underway.

"Once that's done, we will be able to say what strain this is," the health ministry said, adding that the "screening system at airports and entry points is being further strengthened".

The ministry flagged concerns that the location of the confirmed mpox patient in Pakistan is currently unknown, as he is suspected to have gone to another district after providing his samples for tests.

"When we visited his home in Mardan, it was locked from [the] outside and his neighbours told us that the family has left for Dir," the district health office (DHO) said.

"We approached our fellow colleagues of the health department in Dir district but they couldn't trace him, even in Dir," the DHO said.

Clade 1b is said to cause milder symptoms with lesions on the genitals, making it harder to spot and more likely for people to sicken others without knowing they're infected.

All the Walgreens around me are out and the vaccine is currently in back order. Had 2 different appointments cancelled at locations where they supposedly had them in stock. They told me to go back on Wednesday when they're expecting a shipment to arrive. What a mess :deadbanana4:

Philippines detects first mpox case this year, yet to determine strain | Reuters

The Philippines has detected a new case of the mpox virus in the country, the first since December last year, its health department said on Monday, adding it was awaiting test results before being able to determine the strain.

The patient was a 33-year-old Filipino male who had no travel history outside the Philippines, the Department of Health (DOH) said.

"We are awaiting sequencing results and will update once available," its spokesperson Albert Domingo said when asked about the strain.

Pakistan confirms mpox case is not new strain - News | Khaleej Times

One case of the mpox virus has been detected in Pakistan of the clade 2 variety, the country's ministry of health said on Monday, adding that no cases of the clade 1b strain of the disease have been diagnosed.

Clade 1b has triggered global concern because it seems to spread more easily though routine close contact. A case of the variant was confirmed last week in Sweden and linked to a growing outbreak in Africa, the first sign of its spread outside the continent.

Thailand says mpox case recorded in traveller from Africa | Reuters

The director-general did not name the African country the man had been in. He said the man had transited in a Middle Eastern country, which he also did not name, before flying on to Thailand.

Thailand has detected 800 case of mpox Clade 2 since 2022, but so far not detected a case of the Clade 1 or Clade 1b variants.

So I finally got my 1st MPox shot earlier.  Originally I was scheduled to go to CVS, but I called this morning to inquire about getting it at the health department since my state said it was free through them.  I was able to get a 2pm appointment for today.  This meant I could now cancel the one I made at CVS originally.

However, not even an hour after scheduling with the health department, I got an automated text/email from CVS stating my 2:30pm appointment today was canceled with no explanation.  I'm guessing this meant they didn't have the vaccine in stock which is silly considering they make you schedule a week in advance.  The moral of the story, be careful scheduling the vaccine at pharmacies and stick with your local health department if possible. :michael:

I got tested in March and back at that time they were like "you don't have to get this but we have it available because it never went away" so I actually got a third dose of mpox vaccine. Am I good? Idk
