Not that user specifically, but the type of people they get their information from (i.e. lower-level people who work on projects....like the HBO reddit guy). Hundreds of people touch these projects and it only takes one bad apple to screw it up. Also, this was the original image in the code before Chroma iirc - I'm personally happy with the title & cover, but they have been keeping things surprisingly under wrap this time around.

I think for the title there's no way since the website has been done since October last year. The cover being a promo pic doesn't seem impossible especially when we consider that the tracklist was also not found on the website. We will know soon enough though.
