Which begs the question, why did they keep it at #1 despite its poor performance? A few days, sure, but an entire week? Song is still amazing though. They also have a licensing deal with Spotify for their entire catalogue which allows Spotify to host the music in the first place. They are not paying a company that they already own a portion of for playlist spots (which don't even move the needle much anymore). There are 100,000 songs added every single day to Spotify. So Spotify goes to labels to figure out which new music to playlist. They act as a funnel that can help guide Spotify. So when a huge star like Gaga releases a lead single, her label has meetings with Spotify, plays them the song, asks them very nicely to please give it a good playlisting spot. But if the song flops, they're not keeping it at #1 on TTH. Because ultimately for Spotify to even function as a viable streaming platform, they have to cater to what listeners actually want to hear, to an extent. If they just artifically kept songs that are bombing on every playlist, eventually people wouldn't want to listen to those playlists. Just like radio doesn't want to send a song with horrible callout scores to #1. They still need listeners to tune in, by playing music they like. Basically what you're seeing with Disease is just the standard treatment Spotify gives any superstar lead single. Charli was #1 on TTH as well. There's nothing weird or shady about it.
