Teddy is a character that Laeddis invents to ignore his trauma. The entire film premise is a role-play to try and help him but it doesn't work, he still lives in the fantasy (if I remember correctly)

Leo is insane. The facility played along with his delusions for reasons I cannot remember - I think to help him remember he’s insane? I dunno but teddy wasn’t real

What I didn’t understand was why Leo’s character was considered to be this “terrible” person and was sentenced to death, when it was the wife who killed the kids  he just killed her because of that

I chose to believe that he got tricket into thinking he is sick.
