This is why you don't sit or stand under a load that isn't fully secured. A brake, hydraulic jack,... could fail.

He was the one doing the exercise on it and put on extra weights, it probably wasn't properly locked in when he left it. Not only that but after he put on the extra weights he sat on the machine where you are supposed to be standing, him sitting down might have accidentally released the safety on it.

This is terrible but it's probably his own fault, looks like he didn't engage the lock on the squat machine properly and it fell on itself due to the number of discs he'd put on it
