What you've described is not even a difference in "going far enough" but 2 completely different mechanisms that someone would only confuse if completely unfamiliar with how healthcare pricing works. A copay cap does nothing to stop drug companies rom increasing prices, which is what happened In Obamacare's coverage mandates for other various drugs. Insurers then shift the costs that they incur due to the drug companies back to increases on monthly premiums: The actual effective measure, that would actually ensure insulin as a right for 100% of diabetics and not something like 1/5th of all diabetics, would be to cap the actual price that drug companies can sell the drug for.
